Medical records
Your Medical record is a confidential and vital document. It is the policy of this practice to maintain security of personal health information at all times and to ensure that this information is only available to authorized members of staff.
If you wish to transfer any information to another medical practitioner or legal service, kindly make the request in writing. Verbal and Email requests will not be entertained. Further, there may be third party constraints applicable to certain specialist letters and information. Hence, we may need to get clearance from those specialists before the information can be released.
As you are aware, it takes a lot of effort to prepare and maintain patient clinical records, both in written format and in the computer. With most patients, clinical notes run in to many pages. Hence, when a request for transfer of medical record is made, we need to follow the current legal arrangement that stipulates that we send a copy of the record and retain the original. In many situations, the doctors personally have to make copies of relevant notes and letters, for privacy reasons, which is time consuming. This will mean that the patients will need to pay a fee for preparing the copies and posting of the documents to another doctor or legal service.

Procedures
All the doctors at this medical centre have extensive expertise in procedural skills. This involves removal of 'lumps & bumps', skin lesions & cancers, warts or scars, hormone implantations, toe nail surgeries and many more.
In view of the large number of such procedures that are being carried out at this centre, the management has decided to switch over to single use, disposable stainless steel kits instead of the re-usable autoclaved instruments. This would result in a safer and sterile environment during the surgical procedures, ensuring stringent control of 'cross-infection'.
Kindly note that all procedures are billed privately.Patients are requested to settle the accounts on the day of the procedure.

Post Procedure Check List
- The patient may not be able to drive vehicle. Hence speak to your doctor prior to the procedure.
- This may also apply with your ability to work or daily activities
- You will need to keep the body part where the procedure was done clean and dry for few days.
- Daily change of dressings may be required.
- You may be required to return to the doctor after a week for removal of sutures and discuss the report of the Histopathology, if required.
Pathology Results
Pathology, X-Ray Results and Specialist Reports Pathology ResultsWe strongly recommend that you see the doctor to discuss the results of any test.
Kindly take note that it may not be possible to have lengthy discussions of your results over the phone.
The reception staff are not legally bound to give results to the patients. Neither, they are encouraged to access patient's clinical information.Also, no copies of results will be given without prior permission from the doctor.

- The doctor will contact you if there is any significantly abnormal report that needs urgent attention.
- Where there are many results and which may need further discussion, the practice will send a letter or make a call requesting the patient to make an appointment to see the doctor.

Scripts/Referrals
Referral Letters Scripts/ReferralsWe strongly advise that patients should see the doctor for ensuring an appropriate referral letter is made prior they attend a specialist. This may be for the initial or subsequent referrals. The GP may need to update your medical records as well as obtain recent blood / xrays prior referring.
Only in exceptional cases where the patient has attended this surgery within the previous six months for that particular condition, an ongoing referral letter will be made without attendance. Kindly ensure that we are provided adequate notice in preparing this referral letter. However, no back dated referrals will be issued.
This service will incur a minimum fee of $20. Repeat PrescriptionsIt is in the best interest of your health to be seen by the doctor whenever you require repeat prescriptions. This applies especially to blood pressure pills, diabetic pills and antibiotics. In selected cases prescriptions for some continuing medications may be written without consultation. In such situations, a fee of $20 may be charged for a maximum of three items in the script.
Only in exceptional cases where the patient has attended this surgery within the previous six months for that particular condition, an ongoing referral letter will be made without attendance. Sickness Certificates
Sickness certificates will be issued ONLY if the patient has seen the doctor. No back dated certificates will be issued. Please speak to your doctor if you have any concerns.
Similarly, if the parent needs off work note because the child is not well, parent / guardian will need to attend the doctor and proper documentation needs to be completed. Please be aware that all these sickness certificates are "medico-legal" documents. Hence we are compelled to follow the proper protocol as laid out by the Australian Medical Association and other relevant authorities.

We run a Recall and Reminder system for both routine and significant health issues. Some positive aspects of our recall system include:

- We attempt to contact the patient, either by telephone or letter, or both and our reception staff will attempt to update your contact details each time you visit.
